This form is for current members of the LAC Self-Insured Fund.  Submit the following form to update your contact information (mailing address, contact name, contact phone number, etc.).

Inactive members that need to update their mailing address for dividends to be mailed to a new address or to update any other contact information will need to specify they are “INACTIVE” in the notes section of the form.
